Privacy & Patient Care Information
Ontario has a law, the Personal Health Information Protection Act (PHIPA), that protects the personal health information of its residents. We are required to keep your personal health information safe and secure. You have the right to know how we may use it and/or give it out, and how you can have access to it. Medavie Chatham-Kent Ontario will only disclose your personal health information to the appropriate health information custodians, to assist in providing your care.
You and/or your substitute decision maker have the right to:
- View and request a copy of your personal health information;
- Ask us to make corrections to inaccurate or incomplete information; and
- Be told if your personal health information is stolen, lost or improperly accessed.
